The Hunger: High Stakes is an expansion for 2–6 players with a play time of 60 minutes, released in 2022. It adds new twists to the vampire experience through an event deck and a more confrontational approach to hunting.

The event deck creates random starting options, ensuring that each game can begin with a different set of possibilities. These unexpected openings give the expansion a varied setup and add uncertainty from the very start.

Vampires can now attack, bringing new ways to interact with the threats they face and with each other. Attacks allow vampires to defeat new threats, improve their hunting abilities, and take down rival vampires.
