Clinic: Deluxe Edition – 4th Extension is a 2020 board game expansion for 1–4 players, with a listed play time of 150 minutes. It was released for the non-Essen fair in 2020.

This expansion adds new player boards and special tiles to the Clinic Deluxe Edition series. The boards are called basement boards, and each is printed on both sides to support Normal and Expert play.

• 4 new player boards, each printed on both sides
• 9 new very special Double tiles, also printed on both sides
• A dedicated set of rules

The two-sided components provide the expansion’s central additions, with the basement boards offering Normal and Expert sides and the Double tiles featuring different printed faces. The included rules explain how these new components fit into the game.

Its release history is an unusual part of the series. Clinic Deluxe Edition – 2nd Extension and Clinic Deluxe Edition – 3rd Extension were skipped by the designer because of a lack of play testing during the lockdowns, with those extensions planned for release later in 2021–2022.

At the time, Clinic Deluxe Edition – 4th Extension was described as the final expansion and the end of the series. The closing note, however, makes clear that this was not the end after all: “Except it didn’t!” For players tracking the development of the Clinic Deluxe Edition line, this extension is therefore both a substantial component release and an unusual chapter in the series’ publication history.
